£50,000 fine for Tyrone company following death of a worker

Dungannon based firm 'Mid-Ulster Contracts Limited' were fined 50 thousand pounds for health and safety breaches.

These failings led to the death of one of their workers; Newtonstewart man, Phillip Fenwick.

The incident occurred back in 2014 on a construction site on Culmore Road, Derry.

Phillip Fenwick and his co-workers were carrying out a lifting operation but were using a homemade lifting device.

The roof truss they were transporting, fell on his head and he sustained fatal injuries that resulted in his death the following day.

The company were fined as the appropriate precautions were not taken on site and they didn't ensure the safety of their staff.

A HSE inspector, Jonathan Knox, was reported saying that adequate lifting devices must be used when carrying out operations.

The routine must be properly planned and assessed by a competent person, otherwise there's the risk of fatality; which has happened in this case.
